Insurance and Billing

ProScan Imaging accepts most major insurance plans and offers a self-pay option. We’re here to make navigating insurance and billing easier, and we will assist you with understanding your bill. We are committed to price transparency and happy to answer all your questions. Our goal is for you to understand your insurance coverage, required co-pays, co‐insurance, or deductible, and inform you of your financial responsibility before your appointment.

We require payment of the patient portion on the day of your appointment. If you cannot pay the full amount at the time of service, we require a deposit unless previous payment arrangements have been made. Following your visit, ProScan will file claims with your insurance company If the insurance company determines that you owe a balance above any payment made on the date of service, you will receive a bill from ProScan.

Billing Information

Please provide us with complete and accurate insurance information, including your primary insurance plan and secondary insurance plan (if applicable), and a valid contact phone number and home address for patient statements. You will be asked to provide a driver’s license or photo ID and a copy of your insurance card(s) for us to bill your insurance company.

Self Pay

We offer an affordable self-pay option if you do not have health insurance or if you want to opt out of using your health insurance to pay for your test. Click the button below to Request an Imaging Price Quote, and we’ll provide you with transparent pricing before your test. Full payment is due at the time of service. If you cannot pay in full, we offer extended payment plans. If you opt out of using your health insurance, we will NOT file a claim to your health insurance either at the time of service or at a later time. We’re happy to help you decide which option works best for you.

Prior Authorization

Your insurance company may require a pre‐certification or prior authorization for our services. ProScan will coordinate with your referring physician’s office to obtain such authorization from your insurance company. If authorization is not obtained before your scheduled appointment, we will attempt to reschedule. If you choose to have your test performed before authorization or precertification, you may be responsible for payment of the services provided under provisions set forth by your insurance plan. If you have any questions about whether our services will be covered, we urge you to contact your insurance company before your appointment.

Billing & Payment Support

Online Payments: Payments are easy! You can pay your bill online with a Visa, MasterCard, American Express, Discover, and your FSA Card with our secure online portal.

Patient Balances: If a balance is remaining on your patient account after payment is made by your insurance company, you will be billed by ProScan. Payment in full is expected by the DUE DATE stated on the bill. If you need to make payment arrangements, please contact our Billing Office at 866.459.2525 or 513.527.2270 to review other payment options available.

Payment Options: We accept Check, Money Orders, Visa, MasterCard, American Express, or Discover cards.

Refunds: ProScan utilizes a secure, digital-first approach to process refunds on an on-going basis. If you are owed a refund, you will receive an email with a link to activate and access your funds. If your refund has not yet been processed, you may contact the ProScan Billing Department.
