ProScan/NCH Mammo Matching Fund
Mammogram Match Program provides no cost mammograms, breast ultrasounds and other related imaging services at our Women’s Imaging Facility to qualified patients in Collier and Lee county.
To be eligible for the Mammogram Match Program at Proscan NCH Imaging you must meet the following criteria:
- You must be at or below the income guidelines based on current US DHHS Poverty Guidelines
- You must be uninsured or underinsured (HIGH DEDUCTIBLE PLAN)
- You must be a resident in Collier or Lee county
- You must not be receiving services for cosmetic surgery such as pre-breast augmentation
The following supporting documents are required:
- Driver’s license or other form of photo identification
- Copy of medical insurance card (if applicable)
- Pay stubs for the past 30 days for all employed adults living in the same household
- Unemployment check stubs for the past 30 days (if applicable)
- Most recent IRS TAX forms (1040 and W-2)

Early detection saves lives. Getting an annual Mammogram is the best way to detect breast cancer at the early stage when it is curable. Approximately 13% of women will develop breast cancer in their lifetime. We developed this program to make getting your annual mammogram as convenient as possible.
